SSI Evaluations
The SSI Evaluation Process with Dr. Mastikian:

You may call the office to confirm your appointment. We do not return phone calls to confirm appointments.

Your Consultative Evaluation will likely take 30 minutes to an hour to complete unless psychological testing is required. This can take an additional hour or two depending on the tests being requested by the Department.

Please have your picture ID available. Come in prepared with your glasses, hearing aids etc. if you use them on a regular basis and most importantly take your medications as prescribed on the day you come in for your appointment. You will not be seen if these requirements are not met.

Please arrive on time to your evaluation- If you are more than 15 minutes late, we will not be able to assist you due to the high volume of appointments that are scheduled. You will be marked as a ‘NO SHOW’ and you will need to contact your Disability Evaluator to reschedule your appointment. Sometimes accommodations can be made by the doctor who is assigned to conduct the evaluation, but this is dependent on the doctor’s schedule and it is up to the doctor to decide whether an in-house rescheduling will be made.

If you know in advance that you cannot make your appointment and need to reschedule or if you are running late, please contact your Disability Examiner (602-771-7100) or our office at 623-977-1869 as soon as possible.

There is no fee for this evaluation.

Either Dr. Mastikian, Dr. McGady or Dr. Mansfield-Blair will conduct the evaluation; however, the disability examiner and their team will make the ultimate determination as to whether your claim is denied or accepted.

These evaluations are conducted at both of our office locations in Peoria and Flagstaff. Dr. Mastikian also sees claimants at the Tucson DDSA office located on Palo Verde Road in South Tucson.
